$5 trade note from La Fondation Sainte-Jean-du-Millenaire. I believe from 1999. IT IS COMPLETE WITH THE ATTACHMENT ORDER FORM. It will come off as is perforated to leave just the note.  One side has a man in a hat  with a scene in the background and the other side has La Noblesse Du Coeur. Face is green, back is brown. Serial number may be different.
